What is CHISS?
CHISS is a health program administered by the Prince George’s County Health Department

Who does CHISS serve?
CHISS serves people who are 18 years of age or older, who live in Prince George’s County, are low income, and are: Black, Hispanic, or foreign-born; speak English as a second language; have a health condition such as diabetes, asthma, or heart disease; they are older people; and/or live in a ZIP code where the COVID-19 vaccination and/or booster rate is lower than the county average.

What services does CHISS offer?
CHISS will help Prince George’s County residents get health care, health insurance, and assistance with issues like housing, food, utilities, transportation, job training, and employment. The project also educates people on how to stay healthy and avoid getting infected with the COVID-19 virus.

What will you have to do if you join CHISS?
If you join CHISS, you will need to answer questions from one of the Project’s employees, called community health workers (CHWs) or promoters, about the health and social services you need. so they can help you get those services; watch videos and/or read the information they provide to help you improve your health; and meet with them as needed until you are satisfied that you have all the services you need. You must also agree to answer a 20-minute telephone survey about CHISS services if the project investigator calls you.

How does CHISS help participants?
CHISS helps participants obtain the health and social services they and their families need and gives them a gift certificate when they answer questions about the services provided by the Project.
