Social Determinants of Health

Our current program “Health Advocates Promoting Healthy Living” (HAPHL)  aims to mitigate social determinants of health (SDOH), improve healthy eating behaviors, and promote physical activity directly to obese/overweight individuals or those interested in weight management residing in target communities. HAPHL delivers SDOH assessment services, distributes best practices, and culturally and linguistically appropriate healthy eating and physical activity education and information to immigrant, Latino, and African American residents residing in  Montgomery County (MC),  who are at high-risk for poor SDOH outcomes.
